Terry E. Burris, M.D., is a graduate of the University of Kansas School of Medicine. His ophthalmology residency was completed at
the Naval Regional Medical Center in Oakland, CA. and at the University of California / San Francisco.
Dr. Burris performed a research and clinical fellowship in Cornea and External Disease at the McGee Eye Institute of the
University of Oklahoma. He also served four years as Chief of the Cornea Service for the Northern Pacific Rim at the Naval Hospital in Oakland, CA. Afterward, Dr. Burris served four years as the Founder and Chief of the Cornea Service at Devers Eye Institute and continues as a Co-Medical Director of the Lions Eye Bank of Oregon in Portland.

Dr. Burris and his wife, Tima are very involved with their four children. He enjoys family activities, camping, playing the piano,
singing with the church worship team, and running. In 1990, Dr. Burris founded the Northwest Corneal Services clinic to give you the best professional eye health care possible.

Insurance / Financial Information
Medical insurance plans are a private contract
between the patient and the insurance company. As a courtesy to our patients we will bill your insurance, however, you are responsible for the payment of your bill. If your insurance company requires a referral from your primary care physician, you are responsible for obtaining your referral prior to your appointment with Dr. Burris.
Payment of your insurance co-payment is required at the time of your visit.

First Visit
Your first visit will last up to 3 hours. After your
arrival, Dr. Burris or a qualified technician will ask you questions about your eyes and your general health. A series of eye tests will be performed, some of which require eye drops to numb the eye surface. Your eyes may be dilated, so please bring your sunglasses and someone with you who can drive you home.

Please Bring
- A list of all your medications and dosages, including all eye drops used in the last 6 months.
- A list of all allergies (if any).
- All eyeglasses including sunglasses, contact lenses, and low vision aids recently worn.
- Current insurance card.
- Referral from your primary care physician if you are enrolled in an HMO or PPO.

Office Hours
Northwest Corneal Services maintains office hours at several locations to better serve you. We also provide 24-hours a day, 7-days a week care
for you if your referring doctor has determined you have a medical eye emergency. Our regular office hours
are from 8:30AM - 5:00PM, Monday - Friday. For the convenience of our patients, free parking is available.
